- If you click on ” Receipt from purchase or Exchange/return” you are required to enter your 19 digit customer survey code, and click “Start” to continue.
- And, if you click on “Donor Invitation card” here you are required to enter your 4 digit customer code, date and time of your visit, which can be found on your donor card and click “Start” to proceed further.
- After that, you need to answer questions based on store location, products, friendliness of staff, quality of the product, cleanliness of the environment and the overall experience with the company.
- And last you are required to enter your personal information and email ID and submit your survey.
